Testim VS Androgel – Testosterone Replacement Therapy

The two major testosterone gel brands are Testim and Androgel. They are very similar and generally prescribed interchangeably, but sometimes insurance wont cover one or the other. Often, I hear that they wont cover Testim and people want to know if Androgel is just as good. So here is a general guide to choosing if you are lucky enough to have the choice which one is right for you.

Testim Vs AndrogelI would try whichever your insurance covers first. If your insurance covers both medications, try whichever one has coupons or kickbacks. Ive heard people say they didnt pay any/much for Testim for the first year because the company (Auxilium) that markets the drug has a financial assistance program available for the first year.

Now that price and availability are out of the way, lets deal with smell: Testim smells. Some people think it smells good, and some cant stand it. Personally, I like it and my wife likes it. Ive found that women ask me what cologne Im wearing, and mention that they like it too. Still, if you or your wife dont like the smell of something you have to put on every day, that would be a deal-breaker. So try out a tube and see what you and, if applicable, your partner thinks about the Testim scent. Androgel doesnt have much of a smell at all.

Testim is sticky. Androgel goes on much like that hand-sanitizer stuff, while Testim seems to remain stickier longer. Some say this is why Testim seems to work better for them: Because it stays on the skin and soaks in better. These personal statements of efficacy cant be relied on as empirical data, however. But the fact that you can feel it on your skin longer, does bring up a question about Testim gel Vs Androgel: Does it take longer to soak in and, if so, does that increase the exposure time I have to worry about when it comes to my wife and children? The short answer is: NO. In-fact, according to US National Library of Medicines website, the wait-time for AndrogGel is much longer:

You should not shower, bathe, swim, or wash the place where you applied the medication for at least 2 hours after you apply Testim gel or at least 5-6 hours after you apply AndroGel .

WasteTestim comes in little single-use tubes so you go through more of them and it generates more waste. If you are eco-minded this may factor into your decision. Androgel comes in a dispenser with a little pump on it (like soap) so there is less waste.

But the real question when considering AndroGel Vs Testim is: Which works better? Which is the best? 3. Oysters

In research from Finland, scientists gave participants 15 grams of whey isolate both before and after resistance exercises. Muscle biopsy showed an increase in testosterone production of up to 25 percent, which was maintained for 48 hours.13

The authors theorized that a greater expression of testosterone in skeletal muscle could allow for greater uptake from the blood. Although important to the production of testosterone, too much protein can have the opposite effect.

A meat-free diet may lower your testosterone production by up to 14 percent, but that doesn’t mean you should eat excessive quantities of animal protein, either. Excessive protein intake may contribute to the development of chronic diseases like cancer and even accelerate aging.

Consider reducing your protein levels 1 gram of protein for every kilogram of lean body mass, or one-half gram of protein per pound of lean body mass.

7. Garlic

While this fragrant herb doesn’t contain the necessary nutrients to produce testosterone, it does contain allicin, a compound that lowers the levels of cortisol in your body. With your cortisol levels lowered, your body can more effectively and efficiently use the testosterone that is produced.14

Q. Ive been reading about athletes using steroids to build themselves up. Do these drugs work for older men?

A:First some background on steroids. There are two types of steroids: corticosteroids and anabolic steroids. Corticosteroids, such as cortisone and prednisone, are drugs that help control inflammation. Anabolic steroids, such as androstenedione or andro, are substances that can help the body make muscle.

Corticosteroids, which are like hormones that your adrenal glands produce to fight stress, are used to treat arthritis, asthma, lupus, multiple sclerosis, eczema and some kinds of cancer.

Anabolic steroids are drugs that are like the body’s natural sex hormone testosterone. Testosterone directs the body to produce or enhance male characteristics. Medical uses of anabolic steroids include some hormone problems in men, late puberty and muscle loss from some diseases.

When anabolic steroids increase the levels of testosterone in the blood, they stimulate muscle tissue in the body to grow larger and stronger. The effects of too much testosterone can be harmful. Some of the negative effects are rage, liver disease, high cholesterol, severe acne, baldness and infertility.

So-called natural steroids such as DHEA that are sold as over-the-counter supplements at many health food stores can have the same harmful effects as synthetic steroids. The only difference between natural and synthetic steroids is that synthetic steroids are made in a lab and are chemically altered.

There is a lack of available studies on the long-term effects of DHEA. However, DHEA may cause higher than normal levels of androgens and estrogens in the body, and theoretically may increase the risk of prostate, breast, ovarian, and other hormone-sensitive cancers. Therefore, it is not recommended for regular use without supervision by a licensed healthcare professional.

Dont believe advertisements that tell you supplements are natural remedies, implying that they cant hurt you. Some people try supplements such as coral calcium, ginseng and echinacea to stop aging. There isn’t any evidence to support the claims for these supplements either.

Talk to your doctor before taking any supplement. Ingredients in supplements can cause harmful interactions with your medications and serious side effects.

Dear Dr. Roach: I am a 67-year-old male in fair to good health (more good than fair, really). In a recent column, you mentioned that a good testosterone level for a man taking a replacement would be between 500-600 ng/dl.

In November 2016, I was tested for my testosterone level. At the time, I was (and still am) suffering from a low sex drive and erectile dysfunction. My level was 290 ng/dl. The reference range my primary care doctor bases his judgment on has an acceptable range from 193-950 ng/dl; hence he said my level was ”low normal.”

When looking at the symptoms of low testosterone, I noted that I have at least four symptoms: low sex drive, ED (for which I have already been treated with a prosthetic implant), loss of body hair (especially my legs) and, most notably, osteoporosis (for which I take alendronate sodium, 70 mg weekly). I was diagnosed in November.

I also am being treated for depression and anxiety disorder, and have been since 2001. I don’t know if this is related to my testosterone level.

Should I talk to my doctor about the disparity I’ve found in reference ranges? Should I be seeking treatment for the low testosterone? J.P.P.

Dear J.P.P.: I think you definitely should speak to your primary care physician. You also might benefit from a discussion with a urologist or endocrinologist with experience in treating men with testosterone replacement.

When we look at normal testosterone levels by age, we find that older men have lower normal levels; however, given your symptoms and result, I certainly would think a trial of testosterone would be appropriate. I must say that I am surprised that you had an implant placed without a trial of testosterone first. I also am surprised you were treated for osteoporosis without a trial of testosterone replacement, which has been shown to improve bone density in men with low testosterone levels (one study treated men with a testosterone level below 350; another if they were below 320). Low libido and erectile dysfunction both frequently respond to testosterone replacement: Some men get benefit in their mood as well. You sound to me like an excellent candidate for testosterone replacement.

Dr. Roach writes: A recent column from a man asking for alternatives to coronary bypass surgery generated many letters with the same question: Why not advise a change in diet as an alternative to surgery?

There are two reasons. The first is that it’s not an alternative to surgery: It’s a medical recommendation that should be made for every person with diagnosed coronary disease. Nearly all people can improve their diet. While a vegan diet was the most common recommendation I received, it still is not clear that a vegan diet is most likely to reduce coronary disease. In the vast literature on diet, there are only a few well-done studies that show a benefit. The clearest benefit has been from the Mediterranean diet, but a very-low-fat, plant-based diet, in combination with stress reduction and smoking cessation, has been shown to help reverse coronary lesions.

The second reason is that if someone needs the arteries in his or her heart reopened, the changes in diet are not likely to reverse blockages in the time needed to prevent a heart attack.

Healthy diet changes are appropriate for all people with heart blockages, but inadequate by themselves in the short term, in people with symptoms of angina and serious blockages.

As a man the sort of thoughtful, Fawcett Society-supporting man who lowers the toilet seat after peeing, even when he has the house to himself its hard to talk about women and their hormones. Theres no doubt that they affect minds and bodies, through puberty, pregnancy and premenstrual syndrome (PMS). The National Association for Premenstrual Syndromes list of common symptoms includes mood swings, depression, tiredness, anxiety, feeling out of control, irritability, aggression, headaches, sleep disorder, food cravings, breast tenderness, bloating, weight gain and clumsiness.

Men cant and shouldnt ignore this catalogue of woes. But theres a fine line between commiserating and condescending. Its too easy and tempting to dismiss a womans actions or opinions because its that time of the month. Mostly it isnt. Many women are lucky enough to escape PMS. And even when they dont, sometimes shes still right and youre still wrong.

For better or worse, however, we males must now face up to our own fluctuating chemistry. We may not routinely bloat and bleed, but a new study makes it clear that we too are at the mercy of our hormones specifically, the one produced between our legs. After testing hundreds of men, researchers from the California Institute of Technology, Wharton School, Western University and ZRT Laboratory reported (pdf) a clear and robust causal effect of testosterone on human cognition and decision-making.

To put it bluntly, testosterone makes you stupid.

Having persuaded 243 men to smear their torsos with either a placebo or a testosterone gel (imagine the lead-up: No, honestly, were not videoing this No, thats not a one-way mirror No, we are not going to offer you a Diet Coke), the scientists gave them a cognitive reflection test designed to assess their ability to answer tricky questions where their first, intuitive answer was likely to be wrong*. Those who had rubbed themselves with testosterone rather than the placebo answered 20% fewer questions correctly while remaining convinced they were right. They also gave incorrect answers more quickly, and correct answers more slowly than the placebo group. As Caltechs Colin Camerer said: The testosterone is either inhibiting the process of mentally checking your work or increasing the intuitive feeling that, Im definitely right.

If testosterone does encourage hasty decisions, thats not always a bad thing. If youre being chased by a tiger, for example, its better to either run away or improvise a weapon than to just stand there weighing up your options. All the same, this will be a kick in the teeth for big pharma. In the US particularly, drug companies have encouraged doctors to prescribe billions of dollars worth of testosterone supplements to combat the effects of ageing, despite objections that they do nothing for mens health. A host of studies have already shown a correlation between elevated testosterone levels and aggression and now theyre being linked to dumb overconfidence. That wont help with the marketing though it may explain Donald Trump and his half-cocked willy-waggling. Perhaps its not the presidents brain thats running things, but the Leydig cells in his testicles.

Women arent entirely off the hook their bodies also produce testosterone, though in smaller quantities, and the Caltech study notes that it remains to be tested whether the effect is generalisable to females but for now at least they now have another way to fight the scourge of mansplaining: Youre talking out of your nuts.

Better still, with the evils of testosterone firmly established, the world may learn to appreciate older men. Around the age of 30, no longer young, dumb and full of cum, we typically find our testosterone levels declining, so that with every day that passes we become less aggressive, more rational and generally nicer. Your average 54-year-old to pick an age entirely at random can now boast that as well as decades of experience, he can draw on ever-larger reserves of self-control. Whether were trying to land a beautiful lover or a well-paid job, that should make us quite a catch.

According to a story by Catherine Caruso published inSTAT Newsthis week, authorities at theInternational Association of Athletics Federations (IAAF)are getting set to debate whether or not women with hyperandrogenism, or higher-than-expected testosterone levels, should be restricted from competing against women with normal or expected levels. The debate over the IAAF rules began in 2011, when a rule was first created to prevent women with high testosterone levels competing because of the belief that their hormone levels gave them an unfair advantage. The rule was challenged in 2015, and the IAAF was given two years to provide further justification for its position.

As Caruso writes, the main focus of the current controversy is the legal case of Dutee Chand, an Indian athlete whose testosterone levels exceed the 10 nanomoles per liter limit, the level deemed to be the lower end of the male range, i.e., the amount of testosterone in the blood typically exhibited by male athletes. Testosterone is widely considered a hormone that assists in athletic performance, given that it increases the rate of muscle development and bone mass, among other traits. The idea behind the IAAFs position is that unnaturally high levels of testosterone that exceed levels typical of ones gender would give such athletes an unfair advantage over other competitiors. Insofar as the IAAF is concerned with creating the fairest competition possible, the presence of elevated testosterone levels in a select group of athletes, like Chand, presents a serious problem.

The problem with the IAAFs position, however, is that it overlooks one of the central nuances of sporting ethics. It is true that sporting events are supposed to be fair in a wide sense: we would not consider the competition just if one athlete took some action that made it impossible for other athletes to win. This is why athletes are given certain rules to which they must conform. In basketball, for example, one is forbidden from reaching out and grabbing the opposing players arm to prevent them from dribbling; in hockey, players are forbidden from tripping each other; and soccer players cannot decide to randomly touch the ball with their hands (unless, of course, they are a goalie).

Many athletes, regulatory bodies, and ethicists have also long believed that taking certain substances gives an unfair advantage to some competitors if the competition is not also allowed to take them. Two of the most common culprits are testosterone (the hormone for which Chand is in hot water) and human growth hormone (HGH), both of which can aid in athletic performance. Some have suggested that a simple solution might be to allow any athlete to take any substance they want: if there are no limits on what one can take, no substance could truly be considered unfair.

Yet many writers have expressed concern over this view, myself included. The essence of sport seems to be to push oneself to the limits of ones own capacities, to find what one is truly capable of. Bioethicist Maartje Schermer has written that in evaluating the fairness of advantages, rules, and enhancements in sports, we must ask ourselves What exactly do we want to praise or celebrate? In answer, he writes that According to the Presidents Council on Bioethics, what matters most is not even the competitive results but the specific type of human excellence displayed in a sport. This excellence may be thought of as a measurement of what we can naturally achieve through individual effort, which explains why we institute rules prohibiting the use of substances that enhance ones abilities beyond what one could naturally achieve.

The key to the debate over Chand and other athletes with naturally elevated testosterone levels is how we should define natural. While it is true that having an elevated testosterone level is unnatural in the sense that it is not considered typical of a human being of a specific gender, this fact alone does not seem to give an athlete an unfair advantage. If Chand and other athletes were choosing to inject themselves with testosterone to gain an advantage, they would have an unfair advantage because they were actively choosing to elevate their testosterone levels: they would be purposely trying to gain an edge. This conforms to the central criterion Schermer argues constitutes cheating: he writes that Actions that involve the breaking of implicit or unwritten rules and offer an unfair advantage constitute cheating, meaning one must act in order to cheat. For Chand and other athletes, this is simply not the case, and in that sense, they have not done anything unnatural or unfair. Their bodies simply work that way, and while their testosterone levels may not be typical of the average person, we do not normally restrict people from participating in sports just because they happen to be somewhat different than most people. There are many ways that athletes differ from the average person: this is what enables them to perform so well in the first place.

It seems that the IAAF is unfairly targeting a specific naturally-occuring advantage to the exclusion of others. Caruso writes that Georgiann Davis, a medical sociologist from the University of Nevada, has argued that the standards can seem very arbitrary, using the examples of very heavy sumo wrestlers and tall basketball players. Another helpful example might be athletes like Michael Phelps, whose attention deficit hyperactivity disorder (ADHD) and unusual body shape may have given him more energy to train and unique advantages when moving through the water. While these elements gave him a significant advantage over his competition, he was not forbidden from competing because he did not purposely cause these things to occur.

There are certainly appropriate circumstances in which athletic federations are justified in restricting athletes from competing. Athletes like Chand have not purposely broken any rules or elevated their testosterone levels to gain an edge, so they should be allowed to compete.
