Midface Lift Dallas, TX – Mid Face Lift with Dr. Katz

Often, the earliest signs of aging appear in the mid-face region, a triangular area formed between the two corners of the eyes and the corner of the mouth. As time and gravity begin to take their effects, eyelids and cheeks begin to sag, often creating a haggard appearance.

A mid-face lift is a surgical procedure which targets the area around the nose, eyes and corners of the mouth. Midface lifts tighten and smooth the skin and muscle tissue in the cheeks. To perform the plastic surgery procedure, Dr. Katz will create incisions in the temple, in the natural curves in front of the ear and earlobe region. The underlying muscle and connective tissues are tightened, repositioning the cheek fat pads to their desired location.

Dr. Katz performs this procedure on an outpatient basis with IV Sedation and local anesthesia. Surgery typically takes 1.5-2 hours. Makeup can be applied within one week and most patients return to work within one week of surgery.

Wow, I am very sorry this happened to you. This is certainly the worst of the nightmares for any cosmetic surgery patient. You are far enough out from the injury that the only options are static suspension, temporalis muscle transfer or gracilis innervated free flap transfer. None of these are great options and none of them will give you facial symmetry with facial animation. I know you can’t go back in time, but I’m sure you wish you had never had this done. However, I would like to use your example to help others avoid the same problem in the future.

How a Patient Can Avoid Facial Nerve Injury with a Face Lift:

If you are considering facial rejuvenation, facial nerve injury is extremely rare. The quoted rate is way less than 1% and in general is very safe. However, even the “way less than 1%” should never happen. Again, this is a very safe operation and nerve injury should never happen.

I can only speak to my experience, not to say I am the only one qualified to do this operation. I trained in ENT at the Mayo Clinic in Rochester, MN. During residency we worked on and around the facial nerve almost every day for 5 years. This was usually during middle ear surgery, head and neck cancer operations, parotid surgery, facial trauma reconstruction, facial reanimation and more. Generally, this was not for cosmetic purposes, but I learned the landmarks that define the location of the nerve along its entire course. After residency, I participated in a facial plastic surgery fellowship. The director had been in practice for 25 years, performed 1000s of facelifts and had never permanently injured the facial nerve. Today, every time I perform this operation, I think and worry about the nerve the entire time. When we get in the territory where the nerve is found, I stop talking and concentrate on the task at hand. This is true after 2.5 years of practice and hundreds of operations around this nerve. I hope it never changes. So far, no injuries.I know there are many great surgeons out there who can and do safely perform this operation regularly. I am sorry this happened to you. I hope someone can take from your experience, and know how to steer clear of the danger, yet still achieve their goals of facial rejuvenation.

* Injectable skin fillers have added new dimensions to the field of cosmetic dermatology, allowing new forms of facial rejuvenation and wrinkle treatment without surgery. Fillers are designed to increase tissue volume (as with lip augmentation), and improve the skins overall contour.

* Platelet-rich plasma injections help in the generation of healthier cell production and a youthful appearance. Face Lift Patient Before and After 23: This female patient in her late fifties was unhappy with her “turkey neck” (loose neck skin under her chin) and her sagging jowls. Her hair is pulled back and no incision lines or scars are visible.

Natural Laser Mini Face Lift Before and After Patient 32: This woman in her 60s had minimal incision deep plane facelift combined with upper eye lift, lower eye lift with fat transposition, lateral endoscopic brow lift small incisions hidden in the hairline) and fractional laser skin resurfacing. The deep plane facelift is a technique that does not lift the skin but repositions the face under the muscles so the face appears soft and smooth, not stretched. This is what creates natural facelifting results. She has her hair pulled back, and no incisions are visible. Combining this with small changes around the eye and brow, and smoothing of the skin with laser completes her transformation.

In addition to a minimal incision facelift, fat under the chin required removal utilizing liposuction, and a procedure called a platysmaplasty was performed concomitantly to tighten the loose platysma muscle in the neck. The jowls are smoothed and her appearance looks extremely natural.

Lower Face Lift Patient 34: This woman had a minimal incision facelift lower facelift focusing on the jowls and neck. The underlying platysma muscle that hangs under the neck is tightened and the jowl is repositioned so the cheeks look fuller. Her hair is pulled back and no incision lines or scars are visible.

Natural Face Lift and Chin Augmentation Patient 35: This woman had a deep plane face lift requiring additional chin augmentation to rejuvenate the face because of a weak chin. In addition to a minimal incision facelift, a procedure called a platysmaplasty was performed concomitantly to tighten the loose platysma muscle in the neck, and the jawline was enhanced using a chin implant. The jowls are smoothed and her appearance looks extremely natural.

Male Facelift Patient 36: Natural Face lift in a male including Facelift, Lower Eyelid Lift and Rhinoplasty. The face lift rejuvenates the lower third of the face including the jowls and neck. The lower eyelid lift removes the tired look from the eyes. The rhinoplasty brings the face into a better aesthetic balance.

Face Lift Before and After Patient 37: Unhappy with her sagging cheeks, puffiness under her eyes, loose neck region, and deep nasolabial folds, this female patient had Dr. Jacono perform a deep plane facelift procedure to address her concerns. Her after photos show her under eye region no longer looks tired and swollen, her cheeks have been lifted which helped to reduce the appearance of her nasolabial folds. Facelift Patient 45: Mini Type Facelift and Endoscopic Brow Lift. A Mini Face Lift is also referred to as a short scar face lift, S Type Face Lift, J Lift and Life Style Lift. The face lift rejuvenates the lower third of the face including the jowls and neck and the endoscopic brow lift opens the eyes. In this case the patient has asymmetry between the two eyebrows that was corrected.

Facelift Patient 46: Mini Face Lift or S Face Lift. The mini face lift rejuvenates the lower third of the face including the jowls and neck. This surgery was performed under local anesthesia, and the after picture was taken a little over 1 week after surgery.

Face Lift Patient 47: Total Facial Rejuvenation Including Facelift, and Upper and Lower Eyelid Lift. The lower third of the face is rejuvenated, lifting the smile lines, jowls and hanging neck. The upper and lower eyelid lift balances this rejuvenation.

Face Lift Patient 48: Total Facial Rejuvenation Including Facelift, Endoscopic Forehead Lift, Endoscopic Midface Lift, Upper and Lower Eyelid Lift, Lip Wrinkle Dermabrasion, and Chin Augmentation. The upper, middle, and lower third of the face are balanced and rejuvenated with the three separate lifts, lifting the brows, cheeks, smile lines, jowls and hanging neck. The upper and lower eyelid lift balances this rejuvenation.

The upper, middle, and lower third of the face are balanced and rejuvenated with the three separate lifts, lifting the brows, cheeks, smile lines, jowls and hanging neck. The upper and lower eyelid lift balances this rejuvenation.

Face Lift Patient 51: Facial Rejuvenation Including Face lift and Neck Lift, Lip Wrinkle Dermabrasion, and Lip Augmentation. The primary aging change in the cheek area consists of a combination of deflation and descent of the cheek fat pads, creating a hollowed appearance below the lower lids and deepened nasolabial folds (oblique lines which extend from each side of the nose to the area beside the corners of the mouth). Descent of the midface/cheek fat pad can also contribute to the exaggeration of marionette lines and downturning of the corner of the mouth.

This aging change is significant, but is often not fully appreciated as a feature which can age the face considerably. When I see a person who is in their sixties or older but doesn’t ‘look their age’ (but hasn’t had plastic surgery), quite often they will have cheeks which are full and prominent, a feature that the brain immediately associates with youth. Look at an infant!

Cheek area rejuvenation by mean of a mid face lift can be performed as part of a facelift procedure, but is also sometimes performed alone to specifically address deflation and descent of the cheek fat pad. The procedure is performed through two hidden incisions. One is made inside the mouth, at the apex of the space between the gums and the lower cheek. The second incision is hidden behind the hairline of the temple area. Through the intra-oral incision, the cheek fat pad is freed from the cheekbone. A suspension suture is used to grasp the cheek fat pad, and the suture is then passed below the skin to the temple-area incision. Upward traction on the suture elevates the cheek fat pad, and with it the corner of the mouth is elevated slightly. The nasolabial folds soften and in some cases can be almost completely eliminated. The suspension suture is then anchored to the deep soft tissues below the scalp, producing a permanent elevation of the midface.

A technique which does not require incisions or sutures and which I find applicable to more and more patients is structural fat grafting, which involves carefully and painstakingly grafting (by micro-injection) a patient’s own fat into areas which benefit from volume restoration. This is an excellent alternative for facelift patients who have flattening of the midface without significant aging changes in the nasolabial folds or oral commissures (corners of the mouth). In selected patients, I have found this to be a powerful but less invasive means of restoring cheek volume and projection.

Structural fat grafting can also be utilized in patients undergoing a midface lift, if additional enhancement of the lateral aspect of the cheekbones is desired. The midface lift and structural fat grafting, alone or in combination, allow me to restore or create youthful cheek and lower lid contours, without the need for cheek implants. Aside from the associated risks of migration, infection, and bone resorption, I feel that cheek implants tend to produce results that are not natural-appearing, and for that reason I do not use them in any facial rejuvenation procedures.

The Problem

The earliest signs of facial aging often appear in the mid-face area, a triangle defined by the inner and outer corners of the eye and the corner of the mouth. There may be lax and wrinkled lower eyelid skin and fat bags become prominent, oftentimes casting shadows beneath them. The line of demarcation between the lower eyelid and the cheek (lid/cheek junction) begins to drift lower, as time and gravity exert their effects. Making the entire eye area to appear to enlarge. In addition, the cheek drifts downward and begins to bulge, thereby accentuating the naso-labial fold. The net effect is that of “hills and valleys,” rather than a smooth and graceful cheek line.

Background

Traditional facial plastic surgical procedures have been ineffective in correcting all of the problems, even in combined lower eyelid and facelift procedures. The classic lower eyelid tuck could remove excess skin and fat bags, but could do nothing with the lowered lid/cheek junction. The result would often be smooth, taut skin without fat bags but with a hollow-look to the eye. The traditional facelift, or cheek and neck lift, has minimal effect on the midfacial area, including the prominent naso-labial fold. In fact, traditional facial plastic surgical procedures, intent on smoothing skin and removing fat, frequently resulted in taut-skinned, gaunt appearing faces. This, most certainly, was not rejuvenation.

Recent developments

During the late 1990’s, there was a sea of change in the philosophical approach of facial plastic surgery towards treatment of facial aging. We realized that youthful faces were full faces, and that fat itself was not essentially bad. Fat itself was good, although it was oftentimes in the wrong place. Emphasis was begun to preserve fat where possible, to move it to advantageous places on the face and to sculpt it to provide optimal results.

The answer

As a result of these changes in philosophy, a new and sophisticated facial plastic surgical procedure – the midface lift (or microlift)-emerged to address virtually all the problems of the aging eye in one operation. It is performed solely through an incision immediately below the lashes of the lower eyelid and extends slightly down and out at the outer corner of the eye, thereby mimicking a crease line. Through this incision, the entire midfacial triangle of tissue is gently separated from the bone, lifted vertically, and secured to the bone at a higher level. Fat bags may be trimmed if excessive, but most often the fat is preserved and utilized to “fill in” the lid/cheek junction line. Then, the excess skin is carefully trimmed so that it is smooth. Many dissolving sutures are utilized under the skin surface to produce and hold these changes in place. The skin incision itself is closed with fine surface sutures, which are generally removed within 4 – 6 days.

Expectations

The result of a midface lift is a smoother lower eyelid. Fat bags are removed. The lid-cheek junction is both softened (or eliminated) and elevated, thereby avoiding the “operated upon” hollow-eyed look. Thicker tissues of the cheek are placed over the cheekbone, resulting in an enhancement of the cheekbone with the patient’s own tissues. And the naso-labial fold is softened.

How would I look after mid face lift?

One may simulate the results of a mid-face lift by looking in a mirror, placing a finger on the cheekbone just below the outer corner of the eye and very gently elevating the tissues 90% upward and 10% backward. Note that the upper half of the naso-labial fold is improved. The eyelid skin would be smoother and the fat bags would be gone. It is important to understand that a lengthy naso-labial fold, which extends beyond and below the corner of the mouth, will not be totally improved with a mid face lift alone. Also, jowls will not be improved at all. These conditions would require other procedures, oftentimes a facelift.

How is the mid face lift performed?

A mid face lift operation takes from 75 – 90 minutes and is performed on an outpatient basis under anesthesia, administered by a board-certified anesthesiologist. There is no pain or awareness during surgery and the eyes are closed throughout the procedure. Afterwards, there may be soreness but no pain. One is aware of feelings of swelling, pulling and tightness – but there is no pain. There is usually a period of swelling, which lasts 48 – 72 hours, after which the swelling gradually diminishes. There may be bruising, which usually disappears within one to two weeks. There will, however, be continued healing under the skin surface, which may last for three to four months. One should be capable of returning to sedentary-type work – perhaps with some camouflage makeup – within seven to ten days after surgery. One may return to vigorous athletic activities within three to four weeks.

What to expect from mid facelift

The results of a mid face lift are subtle. The object is to rejuvenate the mid facial area, not to change your appearance. The goal is a refreshed, rested, youthful look without a tell-tale “operated-upon” look The mid face will be full, graceful, and continuous, without “hills and valleys.” While the mid face lift does not address all facial areas, when it is combined with a modern two- or three-layer facelift, it adds a certain smoothness to the area around the outer corner of the mouth by providing a vertical lift with the mid face and a lateral lift with the facelift. The net result is that the corners of the mouth and it’s attendant accessory cheek lines, wrinkles and jowls, are all improved to provide a very youthful appearance. For those who have had prior lower eyelid surgery prior cosmetic lower eyelid surgery does not prelude a midface lift. Indeed, a midface lift is often used as a reconstructive or rescue procedure to improve the results of prior surgery, such as inadequate removal of skin, over- or under-removal of fat, a low-placed lid/cheek junction or prominent naso-labial lines. Obviously, a decision for surgery is based upon very careful and individual assessment of the problems presented.
