Depending on how much work is involved, a breast lift procedure takes 1-3 hours. The procedure can be done on an outpatient basis, and it may be possible to go home after the effects of anaesthesia have worn off. You may, however, be asked to stay in hospital overnight to be monitored.

Incisions will initially be red or pink in colour. Over time and with the correct scar management treatment, these scars will fade.

You may not have as much sensation in your nipples and areolas as you had before surgery. This is normal and usually temporary. Sometimes it can take weeks, months or even a year before normal sensation returns.

Although walking as soon as you are able helps prevent blood clots, strenuous activities such as exercise and heavy lifting should be avoided for at least a month.

You may feel discomfort from time to time. This is normal, but if you have severe pain, contact your surgeon.

Two-stage approach to risk-reducing mastectomy improves results for women with large breasts – Medical Xpress

For women undergoing risk-reducing mastectomy to prevent breast cancer, reconstruction can be challenging in those with larger breasts. A two-stage approachwith initial breast reduction and “pre-shaping” followed by mastectomy and reconstructionappears to be a safer procedure with better cosmetic results, reports the September issue of Plastic and Reconstructive Surgery, the official medical journal of the American Society of Plastic Surgeons (ASPS).

Targeted breast reduction/pre-shaping “primes” the breast for mastectomy and implant reconstruction performed a few months later, according the study by Gudjon L. Gunnarsson, MD, and colleagues of Telemark Hospital, Skien, Norway.

Breast Pre-Shaping Lowers Risks, Improves Final Results

More women are undergoing mastectomy and breast reconstruction to reduce their risk of familial breast cancer, based on detection of specific risk genes. Typically this is done in a single-stage procedure, with “nipple-sparing” mastectomy of both breasts followed by immediate breast reconstruction using implants.

However, it can be challenging to achieve good results with immediate reconstruction in women with large breasts (macromastia) with sagging or drooping (ptosis). These patients “more often end up having a difficult corrective procedure and unacceptably high rate of failed reconstruction,” Dr. Gunnarsson and coauthors write.

To improve their results, the researchers developed a targeted, two-stage approach to this group of patients. In the first stage, the women underwent a breast reduction/breast lift (mastopexy) procedure to create “a breast better suited for implant reconstruction.” In addition to reducing and reshaping the breast, an important part of this procedure was to lift the nipple-areola complex (nipple and surrounding tissues) to a more central position.

After a few months for healing, the women proceeded to the standard risk-reducing mastectomy and implant-based reconstruction. The online version of the article on the Plastic and Reconstructive Surgery website includes a video illustrating key aspects of the technique for plastic surgeons.

Between 2013 and 2015, Dr. Gunnarsson and colleagues used this two-stage approach in 22 women, median age 46 years. Mastectomy and subsequent reconstruction were successfully completed in all 44 breasts. The reconstructions were performed three to nine months (median four months) after initial breast reduction/reshaping.

Two patients had complications requiring repeat surgery; another five patients had minor complications that resolved with nonsurgical treatment. Importantly, there were no problems with the blood supply and survival of the nipple-areola complex.

In all cases, the cosmetic outcomes were rated excellent. The authors believe their technique is an important contribution to improving the results of risk-reducing mastectomy and breast reconstruction for women with large breasts. They note that the initial breast reduction/preshaping step “preconditions” the blood supply to the later reconstructed breast.

Dr. Gunnarsson and colleagues add that the two-stage procedure has become their “standard of care” for women with large breasts undergoing risk-reducing mastectomy. The researchers write, “The superior cosmetic outcome and low complication rate have proved to be better than we had anticipated.”

Cosmetic Surgery May Help Patients Quit Smoking – Newswise (press release)

Newswise August 29, 2017 If you’re a smoker considering cosmetic surgery, your plastic surgeon will likely require you to stop smoking for at least two weeks before your procedure. A long-term follow-up study finds that many patients receiving these instructions will quit smoking, or at least smoke less, in the years after cosmetic surgery, reports the September issue of Plastic and Reconstructive Surgery, the official medical journal of the American Society of Plastic Surgeons (ASPS).

“Our results show an association between cosmetic surgery and smoking cessation at long-term follow-up,” comments lead author Aaron C. Van Slyke, MD, MSc, of University of British Columbia, Vancouver. “Surgeons who request preoperative smoking cessation may influence patients’ long-term smoking status.”

Many Cosmetic Surgery Patients Quit or Reduce Smoking

The follow-up study included 85 patients who were smokers when evaluated for cosmetic surgery. Like most plastic surgeons, Dr. Van Slyke and colleagues required patients to refrain from smoking for at least two weeks before elective procedures. Those instructions reflect a well-demonstrated increase in wound healing problems and other negative outcomes among smokers after plastic surgery.

Five years after cosmetic surgery, 47 patients responded to a follow-up survey. Most of the patients were women; the average age was 40 years. The most common procedures were “tummy tuck” (abdominoplasty), breast lift (mastopexy), and facelift. After exclusion of five “social” smokers, the study included 42 patients who were daily smokers before cosmetic surgery.

In the follow-up survey, about 40 percent of patients said they no longer smoked cigarettes on a daily basis. Nearly one-fourth had not smoked at all since their cosmetic surgery procedure.

Most patients said they had reduced their cigarette use by any amount. Seventy percent agreed that discussing their increased surgical risks with the plastic surgeon influenced their ability to quit or reduce smoking.

However, one-half of patients admitted they did not follow the instructions to refrain from cigarette use before surgery. Nearly one-fourth continued to smoke up to the day of their procedure. Dr. Van Slyke and colleagues did not test to confirm whether patients had stopped smoking.

The complication rate after cosmetic surgery was higher in patients who continued to smoke: 24 percent versus 14 percent. (The difference was not statistically significant.) More serious wound-healing complications occurred in two patientsboth of whom did not follow the instructions to stop smoking.

Due to the negative effects of smoking on wound healing, many plastic surgeons are unwilling to perform cosmetic surgery procedures in patients who smoke. Compared to studies in other groups, the new findings suggest that cosmetic surgery patients seem more likely to quit or cut back on smoking at long-term follow-up. “This is consistent with previous research showing patients who seek to obtain cosmetic surgery are more motivated to sustain positive lifestyle changes,” Dr. Van Slyke and coauthors write.

The results suggest that patients were more motivated to quit by “targeted messaging” with specific examples of the negative effects of smoking, rather than by a description of the general health benefits of smoking cessation. The authors conclude, “[T]he dialogue between plastic surgeon and patient during the cosmetic surgery consultation serves as a unique moment to provide targeted smoking cessation counseling that may persist well beyond the surgical interaction.”

A breast lift is not a simple operation, but its normally safe when performed by a qualified plastic surgeon. Nevertheless, as with any surgery, there is always a possibility of complications or a reaction to the anaesthesia. Bleeding and infection following mastopexy are uncommon, but they can cause scars to widen. You can reduce your risks by closely following your surgeons advice both before and after surgery.

Mastopexy does leave noticeable, permanent scars, although theyll be covered by your bra or bathing suit. (Poor healing and wider scars are more common in smokers.) In your initial consultation, I will examine your breasts and measure them while youre sitting or standing. I will discuss the variables that may affect the procedure such as your age, the size and shape of your breasts, and your skin condition and whether an implant is advisable. We will also discuss where the nipple and areola will be positioned; theyll be moved higher during the procedure, and should be approximately even with the crease beneath your breast.

Depending on your age and family history, your surgeon may require you to have a mammogram (breast x-ray) before surgery. Youll also get specific instructions on how to prepare for surgery, including guidelines on eating and drinking, smoking and medication.

While youre making preparations, be sure to arrange for someone to drive you home after your surgery and to help you out for a few days if needed.

Mastopexy usually takes one and a half to three and a half hours. Techniques vary, but generally involve an anchor-shaped incision following the natural contour of the breast, or an incision around the nipple and extending to the groove beneath.

The incision outlines the area from which breast skin will be removed and defines the new location for the nipple. When the excess skin has been removed, the nipple and areola are moved to the higher position. The skin surrounding the areola is then brought down and together to reshape the breast. Stitches are usually located around the areola, in a vertical line extending downwards from the nipple area, and sometimes along the lower crease of the breast. Incisions outline the area of skin to be removed and the new position for the nipple. Skin formerly located above the nipple is brought down and together to reshape the breast. Sutures close the incisions, giving the breast its new contour.

After surgery, the breasts are higher and firmer, with sutures usually located around the areola, below it, and in the crease under the breast.

Some patients, especially those with relatively small breasts and minimal sagging, may be candidates for modified procedures requiring less extensive incisions. For example, the vertical mammaplasty leaves a scar around the nipple and from the nipple to the groove below.

If youre having an implant inserted along with your breast lift, it will be placed in a pocket directly under the breast tissue, or deeper, under the muscle of the chest wall. Back to Top

After surgery, your breast will be supported with adhesive tape and dressings. Your breasts will be bruised, swollen, and uncomfortable, but the pain shouldnt be severe. Any discomfort you do feel can be relieved with tablets prescribed by your surgeon.

After a few days, you can bathe and at about one week the tape will be replaced by a soft support bra. Youll need to wear this bra around the clock for about three weeks. The stitches are usually absorbable and wont need removing.

You can expect some loss of feeling in your nipples and breast skin. This numbness usually fades as the swelling subsides over the next six weeks or so, but in some patients, it may last a year or more, and occasionally it may be permanent. Back to Top

Healing is a gradual process. Although you will be up and about in a day or two, dont plan on returning to work for a week or more, depending on how you feel.

Avoid lifting anything over your head for three to four weeks. If you have any unusual symptoms, dont hesitate to call. You will be given detailed instructions for resuming your normal activities.

You may be instructed to avoid sex for a week or more, and to avoid strenuous sports for about six weeks. After that, you can resume these activities slowly. If you become pregnant, the operation should not affect your ability to breast-feed, since your milk ducts and nipples will be left intact. Back to Top

Every effort will be made to make your scars as inconspicuous as possible; but remember that mastopexy scars are extensive and permanent. They often remain lumpy and red for months, then gradually become less obvious and eventually they become pale. Fortunately, the scars can usually be placed so that you can wear even low-cut tops.

You should also keep in mind that a breast lift wont keep you firm forever the effects of gravity, pregnancy, ageing and weight fluctuations will eventually take their toll again. Women who have implants along with their breast lift may find the results last longer.

How do I know if I should have a breast lift, implants or both? Consider these factors when making this decision:

Arent implants alone enough to correct breast sagging? Implants alone can improve the appearance of breast sagging in women with a very mild degree of ptosis. However, there is a tendency for some to recommend a larger implant in this situation to try to overcome the ptosis. But then the breast becomes, in my opinion, too large for the patients torso. My approach is to begin by asking, What is the best breast size for the patient? and then asking, How do we deal with the breast tissue to have it in an improved position?

What do you think distinguishes your approach to breast lifts? Like everything else we do, we try to see what will be the best answer to the patients problem, and then suggest a procedure for the problem as opposed to trying to find a problem for the procedure.

Can exercise lift my breasts? Exercise, diet and creams will not change the lift in your breasts. A breast cannot contract to its original size without surgical intervention.

Im still thinking about having more kids. Should I wait to have a breast lift? My advice would be to wait to have a breast lift until you have completed your family because pregnancy can change the shape of the breasts and may tend to undo some of the results of the surgery. However, should you subsequently become pregnant an additional procedure can further restore the shape of your breasts

Will a breast lift affect my ability to breastfeed? Probably not. This is more of an issue with breast reduction surgery, but Ive had plenty of breast reduction patients who have been able to breastfeed.

Why do some women choose to have their implants removed during a breast lift? Women who have had breast implants for many years may find their body image has changed. If a woman has gained a few pounds as she has matured, she may feel she looks more matronly with larger breasts. Removing the implants and performing a breast lift has the same effect as a breast reduction it gives the woman a more uplifted, youthful look.
